Three benefit levels to choose from.  Summary of the Diamond level is below*:

 

- Indemnity plan, ages 18-64

 

- $1,000 per day hospital benefit, 100 days annually plus additional  

 $1,000 per day for ICU, 5 days annually

 

- No lifetime maximum benefit limit

 

- Any doctor/any hospital or PPO network MultiPlan has over 500,000

 providers in the U.S.

 

- Surgical benefit inpatient/outpatient 100% medicare reimbursement

 

- Wellness benefit $150 per person per year, x-ray & labs covered

 

- 5 doctor office visits per year, $50 or $75 depending upon plan level*

 

- Discount prescription drug card included

 

- Optional generic prescription copay card, $1,500 annual cap per
   person, 53,000 pharmacies in the U.S.

 

- Accident and Critical illness benefits at different levels depending
  upon plan selected

Health Choice - Minimal Underwriting

 

Two benefit levels to choose from.  A summary of the Premier level is below*:

 

- Only 6 medical questions

 

- Indemnity plan, ages 18-64

 

- $1,000 per day hospital benefit; $2,500 for 1st day, 100 days annually,

 plus additional $1,000 per day for ICU, 15 days annually

 

- No lifetime maximum benefit limit

 

- Any doctor/any hospital or PPO network MultiPlan has over 500,000
   providers
in the U.S.

 

- Surgical benefit inpatient/outpatient 100% medicare reimbursement

 

- Wellness benefit $100 per person per year, x-ray and labs covered
   at $250 per visit, 3 visits per year

 

- 5 doctor office visits per year, $50 or $75 depending upon plan level*

 

- Discount prescription drug card included

 

- Optional generic prescription copay card, $1,500 annual cap per
   person, 53,000 pharmacies in the U.S.
